We are reaching out to you as a health professional to assist us in spreading the message about the benefits of physical activity for women. You play an important role in educating your patients, community-members and friends about their risks for heart disease and stroke. A health professional is defined as a person with a degree or certification in one of the following disciplines: medicine, nursing, nurse practitioner, exercise physiology, nutrition, physical therapy, rehabilitation therapy, personal training, health promotion or health education. A recent survey conducted for the American Heart Association shows that less than 1 in 4 women consider physical inactivity a major cause for developing heart disease. We need your help to change this perception and motivate women to take action! As the health professional team leader, you will select your team members and meet as a group at least four times during the Heart disease in women Center 12-week program. A minimum of five persons is required for a team. (There is no maximum limit.) At the initial team meeting, we ask you to conduct the following assessments:
