Health Professional Team Component

We are reaching out to you as a health professional to assist us in spreading the message about the benefits of physical activity for women. You play an important role in educating your patients, community-members and friends about their risks for heart disease and stroke. A health professional is defined as a person with a degree or certification in one of the following disciplines: medicine, nursing, nurse practitioner, exercise physiology, nutrition, physical therapy, rehabilitation therapy, personal training, health promotion or health education.

A recent survey conducted for the American Heart Association shows that less than 1 in 4 women consider physical inactivity a major cause for developing heart disease. We need your help to change this perception and motivate women to take action! As the health professional team leader, you will select your team members and meet as a group at least four times during the Heart disease in women Center 12-week program. A minimum of five persons is required for a team. (There is no maximum limit.) At the initial team meeting, we ask you to conduct the following assessments:

" Height
" Weight
" BMI
" Blood Pressure
" Cholesterol

This will provide baseline data to your team on their health status. In addition, we ask you to meet with your team at least once a month and plan an activity that will augment the messaging in the
Heart disease in women Center
handbook. Use the "Move and Groove" Activity Sheet for suggestions. Enrollment is EASY!

2. Recruit your team.
3. Complete and return the registration form and team profle to us via fax at (214) 706-5244.
4. We (Heart disease in women Center) will send you registration packets for each team member.
5. You will be responsible for distributing, collecting and mailing the completed registration forms for the team to the address below.
6. We will send you the program handbooks for each team member.
7. At the end of the 12-week program, you will collect and mail-in the post program forms for the team to the address below. We will send you a gift for each person that completes the program.

That's it! In no time, your team will be enjoying the benefits of being more active and working together!
